29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a.) On or about January 28, 2025 and at times prior, in welding bay #4, multiple extension cords with cuts exposing internal wiring were in use in building #1. b.) On or about January 28, 2025 and at times prior, in the middle building #7, strain relief was not provided on an extension cord used in the shop. c.) On or about January 28, 2025 and at times prior, on the north end of building #7, strain relief was not provided on an extension cord used in the shop. d.) On or about January 28, 2025 and at times prior, in enclosure #6 of the pipe shop, strain relief was not provided on an extension cord used in the shop.
