On or about July 28, 2014 the employer did not ensure that protruding rotating shafts that did not have smooth edges were guarded so as to prevent accidental contact with employees exposing them to caught-in hazards in the following instances: a) Underneath bins on a conveyer belt b) Underneath bins on a bin conveyer system
