29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human fac tors: The employer does not ensure that the process hazard analysis human factors chec klist is appropriate for the complexity of the process. a) This violation was do cumented on November 18, 2010, where the process hazard analysis, human factors checklist was not appropriate for the complexity of the process when evaluating the manpower necessary to implement an effective mechanical integrity program. T his exposed employees to the hazards of fires and explosions. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, in cluding describing the steps it is taking to ensure that the process hazard anal ysis human factors methodology is modified to address the complexity of human fa ctors specific to the facility.
