29 CFR 1910.216(b)(1): An accessible safety trip control that operated on contact was not provided in front and/or back of each mill: North Building, Holding Mill: On, or about, October 14, 2022, the emergency stop cable provided on the front of the mill was in line with a safety bar that was installed to prevent accumulated material from falling off the top of the mill. The position of the safety bar/emergency stop cable could prevent the cable from moving enough to trip the emergency stop circuit in the event of an emergency.
