29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: a. On the Roof - On or about February 9, 2022, the employer failed to inform employees of the hazards of confined space through the use of signs or other messages on the kill room air handling unit, exposing employees to caught in and ammonia hazards. b. On the Roof - On or about February 9, 2022, the employer failed to inform employees of the hazards of confined space through the use of signs or other messages on the Accuar 1 unit, exposing employees to caught in and ammonia hazards. c. On the Roof - On or about February 9, 2022, the employer failed to inform employees of the hazards of confined space through the use of signs or other messages on the Accuar 2 unit, exposing employees to caught in and ammonia hazards. d. On the Roof - On or about February 9, 2022, the employer failed to inform employees of the hazards of confined space through the use of signs or other messages on the air duct filter ports, exposing employees to caught in and ammonia hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
