29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about July 23, 2019, the employer failed to ensure the that energy control program and procedures were inspected annually. There was no annual review of the program. Employee(s) were exposed to such hazards such as, but not limited to electric shock, fractures, burns and inhalation hazards from the unexpected start-up and/or un-expected release of energies and/or chemicals.
