29 CFR 1910.146(k)(2)(iv): The employer did not ensure that affected employees practiced making permit space rescues at least once every 12 months, by means of simulated rescue operations in which they remove dummies, manikins, or actual persons from the actual permit spaces or from representative permit spaces: a) At the facility: On or about August 17, 2018 and times prior to, where employees were exposed to inhalation and asphyxiation hazards when entering tanks for cleaning without the practicing rescue from a representative permit space such as an IDLH atmosphere.
