29 CFR 1910.219(i)(2): Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve: On or about October 26, 2016, in the casting department, employees processing the resin mix at the mixing stations were exposed to the unguarded, rotating coupling shaft above the mixing barrel. Employees exposed to the hazard of being caught-in the rotating coupling shaft.
