29 CFR 1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts: a/ The employer is failing to protect employees from arc flash and or electrocution hazards. This condition was last observed on and about 05/05/16, at 16051 MO-HWY 47, Marthasville, MO 63357. Inside of Building #1, three General Electric 60 AMP Disconnect Boxes are installed on a live, 480 volt, Middle Row Bus Duct. Appropriate insulated tools are neither available nor used to protect the employee from this hazard.
