Falls Creek Powdered Metals, Inc., 82 Industrial Drive, Brockway, PA, 15824, on or about November 23, 2015 and times prior thereto: Employees are provided with and permitted to utilize portable fire extinguishers including, but not limited to, the Badger 10-lb. dry chemical fire extinguisher (M/N: 10MB-8H, S/N: V-413578). Employees have not been trained by the company.
