29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: On or about September 14, 2015, pallet jack operator had not been provided with refresher training after an employee was injured in an incident in which the employee struck his knee on a pallet jack or the pallet jack struck the employee's knee. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
