29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. Jobsite: 1. Bldg. A, front unit exterior: The alum extension ladder at the front of the building, used by employees to access the 2nd floor via the window opening, did not extend 3-ft beyond the opening. 2. Bldg. A, rear unit 2nd floor: The 8-ft stepladder, used by the employees to access the 3rd level deck, did not extend 3-ft above that deck. 3. Bldg. A, rear unit, 1st floor stairway landing: The folded 8-ft stepladder on the 1st floor stair landing, used by employees to access the 2nd floor, did not extend 3-ft above the second floor. 4. Bldg. B, front unit 2nd floor: The 6-ft stepladder, used by the employee to access the 3rd level deck, did not extend 3-ft beyond the deck. 5. Bldg. B, rear unit, 2nd floor: The 8-ft stepladder, used by the employee to access the 3rd level deck, did not extend 3-ft beyond the deck.
