Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the CNC Building; a relocatable power tap (RTP) was daisy chained and left in a pile of dust along the east wall of the facility adjacent to a computer desk, creating a potential fire hazard. b) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin the Maintenance Shop; two relocatable power taps (RTP) were daisy chained and affixed to a portion of metal conduit with zip ties, creating a potential fire hazard. c) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in Aluminum Building; a relocatable power tap (RTP) was used to power three potentially high draw appliances, including but not limited to a microwave, refrigerator and a heated water cooler, creating a potential fire hazard. d) On or about February 15, 2023, at a workplace located at 210 Maple Street, Denmark, Wisconsin in the Cheese Box Area; a metal handibox was connected to flexible cord, being used as a pendant and not permanently affixed to a stable structure.
