29 CFR 1910.212(a)(4): Barrels, containers, and drums. Revolving drums, barrels, and containers shall be guarded by an enclosure which is interlocked with the drive mechanism, so that the barrel, drum, or container cannot revolve unless the guard enclosure is in place: a. On or about February 28, 2018, located in the mill room, employees worked at and near a rotating barrel in a rinse tank that was not guarded with an interlocked enclosure, thereby exposing the employees to a caught-in hazard. 29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: a. On or about February 28, 2018, located in the Mandrel Hose "Geese" Finishing Room, employees operated eight pneumatically-driven swing saws that had inadequate guarding over their blades, thereby exposing employees to being stuck-by the cutting blade of the saw. b. On or about February 28, 2018, located at the Banbury Rubber Mill, employees cut by hand and repositioned rubber material while there was an affixed attachment with two unguarded cutter wheels, thereby exposing the employees being caught in the point-of-operation. c. On or about February 28, 2018, located in the Finishing Department on #1 Cutter, employees operated the cutter in order to cut rubber material with inadequate guarding that did not protect the employees from making contact with the point of operation. d. On or about February 28, 2018, located in the Finishing Department on #2 Cutter, employees operated the cutter in order to cut rubber material with inadequate guarding that did not protect the employees from making contact with the point of operation. e. On or about February 28, 2018, located in the Finishing Department on #4 Cutter, employees operated the cutter in order to cut rubber material with inadequate guarding that did not protect the employees from making contact with the point of operation. f. On or about March 21, 2018, located in the Vucanizing Department, employees operated 12 foot-actuated pneumatic Thomas Bus presses that had no guarding where the dies come in contact with the rubber material, thereby exposing the employees to a caught-in hazard. g. On or about March 21, 2018, located in the Extrusion Room on Hot Extruder Line #3, employees operated a water supplied cutting saw that had inadequate guarding to protect the employees from making contact with the point of operation. h. On or about April 10, 2018, located in the Banbury where rubber cutting was performed, there was a pneumatic shear with inadequate guarding that employees operated, thereby exposing the employees to the point of operation of the blade.
