29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about February 21, 2017, in building 2, during the fabrication of the hunting blinds, employees were plugging flexible cords into outlets where the receptacles were missing face plate covers and internal, energized conductors were present. The employees were exposed to an electrical shock hazard. a) Table saw area, there were three 120-V junction boxes being used to plug in drills that were missing face plate covers. b) Welding station at the entrance of the building, there was a 400-V receptacle being used for the welder that was missing a face place.
