29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about August 11, 2016, and times prior thereto, at this establishment, employees used a wall mounted duplex receptacle electrical outlet boxes as an electrical multiple outlet to connect an extension cord and energize equipment, and the electrical outlet boxes were laying on the floor and not mounted in accordance with manufacture instructions: a. Wall mounted duplex receptacle electrical outlet box connected to a fan; and, b. Wall mounted duplex receptacle electrical outlet box connected to another piece of equipment.
