29 CFR 1910.252(b)(4)(iv): Where the welder must enter a confined space through a manhole or other small opening, means was not provided for quickly removing the welder in case of emergency: On or about October 2, 2014, the employer allows employees to enter tank cars to weld and grind without providing means for removing the welder in case of emergency. Employees were exposed to hazards associated with inhalation hazards.
