29 CFR 1910.27(b)(1)(ii): The distance between rungs, cleats, and steps shall not exceed 12 inches and shall be uniform throughout the length of the ladder. (a) Cook Plant - On or about September 26, 2012 a fixed ladder in a 10 to 15 feet deep pit had 2 missing rungs. Each rung was 10-12 inches apart exposing employees to the hazard of falling to the bottom of the pit.
