29 CFR 1910.119(l)(2)(ii): The procedures did not assure that the impact of the change on safety & health was considered prior to the implementation of the chan ge. This violation was observed on or about 1/19/2010, where the employer did no t ensure that safety & health issues were addressed during the review of propose d changes to the equipment, structures, etc as listed below on the EC2 platform. This exposed employees to fire, explosion and chemical exposure. Instances: a.M OC #283 dated 04/16/07 - installation of a back pressure valve on natural gas compressor outlet. b.MOC #475 dated 03/04/08 - installation of Callon production equipment on platform c.MOC #531 dated 05/21/08 - installation of deck extensio n & 8 man sleeping quarters.. d.MOC #1135 dated 11/10/09 - Rerate MBD-0500 HP Te st Separator from 1440 to 1350 psi e.MOC #1348 dated 04/02/10 - Replace damaged/ destroyed equipment. Pursuant to 29 CFR 1903.19(d), within 10 calendar days of t he date of the citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps it is taking to ensure that all potential safety & health hazards will be evaluated for all M OC's generated and documented in the platforms' records.
