29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices: (a)Minimum required thicknesses were not appropriately established for process piping per API 574, Inspection Practices for Piping System Components, Section 11.1.4. (b)100% radiography or ultrasonic testing was not conducted on process piping weld repairs per ASME B31.3, Process Piping, Sections 341.4.3 and 345.9.1. (c)There was no Fitness-For-Service assessment for Tank U-613 which underwent repairs resultant to cracking at the effected area of the east head 2" nozzle where said cracks were ground out and repair welded per API 579, Fitness-For-Service, Sections 2.1.3, 9.2.2.1, and 9.4. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
