29 CFR 1910.119(e)(3)(vi): The process hazard analysis (PHA) did not address human factors: The employer failed to develop findings and recommendations for the following: b- Hess Port Reading Refinery, Gas Con - The 2004 PHA failed to consider the time needed by operator(s) to perform emergency procedure 15-6.503 Loss of Instrument Air which requires operator(s) to close an extensive number of control valves or verify their fail safe position. Condition noted on or about 03/05/09. d- Hess Port Reading Refinery, Alky - The 1999 PHA failed to develop findings/recommendations when a "no" response was generated to the Human Factors Question "Is all-important equipment (vessels, valves, instruments, controls, etc.) clearly and unambiguously labeled?" Condition noted on or about 02/27/09.
