29 CFR 1926.1204(h):The employer did not designate each person who was to have an active role in entry operations and did not identify the duties of each such employee, and did not provide each employee with training required by 1926.1207. a) Williamstowne Curve (Access Road) Mobile, Al, on or about August 11, 2025: employees entered a sewer manhole containing a hazardous atmosphere to perform work without the roles of the employees involved in the entry operation being designated by the employer.
