Bldg 31C20T, Temporary Fabric Shelter, LCAC 81: On or before March 14, 2024, employee(s) were tasked to perform cleaning operations in the fuel tank(s) 1-4, overflow tank, and oily waste tank. The employer failed to document an occupational hazard assessment of fuel tank cleaning operations. Employee(s) were potentially exposed to fire hazard(s), flammable vapors, and toxic atmosphere.
