29 CFR 1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: a) International Cushioning Co. on or about February 22, 2022, at the address of 2101 Cedar St. Fremont, OH: The employer did not ensure the junction box containing 480 volt conductors atop the motor of the recycling line hopper was fully enclosed. Employees operating near this condition were exposed to electrical shock hazards.
