29 CFR 1910.334(a)(2)(ii):If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. On or before 2/7/22 the employer did not ensure that damaged or defected electrical equipment were removed from service due to an electrical hazard such as missing outlet cover, exposed wires, damaged power supply cords and missing buttons on control boxes, on machine such as: a) On or about 1/17/22 the employer did not ensure that defective equipment such as the Havir Press #2 (model # SE 3963) were removed from service in that a metal electrical supply box was cutting into the power supply cord therefore exposing employees to injures. b) On or about 1/31/22 the employer did not ensure that defective equipment such as The V&O Press Co. (model # missing) were removed from service in that a button was missing from the control panel. c) On or about 2/7/22 the employer did not ensure that defective equipment such as Pines Teledyne pipe bender (model # 1LS) were removed from service in that a power button was missing from the control panel exposing employees to a shock hazard. d) On or about 1/17/22 the employer did not ensure that defective equipment such as Reid Cat # 956 (model # 08434) were removed from service in that the power cord had a cut in exposing employees to injures. e) On or about 2/2/22 the employer did not ensure that defective equipment such as Wesco Knitting machine #6 no model number were removed from service in that the machine has exposed power supply wires of greater than 50 volts, exposing employees to injures. h) On or about 1/17/22 the employer did not ensure that defective equipment such as Wesco Knitting machine #13 no model number were removed from service in that the machine has exposed power supply wires of greater than 50 volts, exposing employees to injures. i) On or about 2/1/22 the employer did not ensure that defective equipment such as Wesco Knitting machine #14 no model number were removed from service in that the machine has exposed power supply wires of greater than 50 volts, exposing employees to injures. j) On or about 12/31/21 the employer did not ensure that defective equipment such as Wesco Knitting machine #10 no model number were removed from service in that the machine has exposed power supply wires of greater than 50 volts, exposing employees to injures. k) On or about 2/2/22 the employer did not ensure that defective equipment such as Wesco Knitting machine #1 no model number were removed from service in that the machine has exposed power supply wires of greater than 50 volts, exposing employees to injures.
