29 CFR 1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: a) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Workers were exposed to electrical shock hazards while they were performing maintenance using the red/black Southwire extension cord storage reel that had damaged integral wiring and was missing its ground pin. Violation observed on or about February 1, 2022. b) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A worker was exposed to electrical shock hazards as the tig welder that was used in the Machine Repair Shop had a damaged power cord due to a broken strain relief where it connected to the machine. Machine was a Pulse / MMA Inverter - TIG 350T welder. Violation was observed on or about February 1, 2022. c) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - A worker was exposed to electrical shock hazards as the tig welder that was used in the production areas had a damaged power cord due to a broken strain relief where it connected to the machine. The welder's 220V cable was also badly damaged in the main cable as well as having no strain relief to the plug. Machine was a TIG Handy 300ST welder. Violation was observed on or about February 1, 2022. d) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Workers were exposed to electrical shock hazards while they were performing maintenance using the orange Link2Home extension cord storage reel that was missing its ground pin. Violation was observed on or about February 1, 2021. e) Worksite was located at 2 Applegate Dr, Robbinsville Twp, NJ 08691 - Workers were exposed to electrical shock hazards while they were performing maintenance using the large circular fan identified as "Terranos 10481883" that was missing its ground pin. Violation was observed on or about February 1, 2021. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
