29 CFR 1915.132(d): Worn, frayed and or damaged electric cables were not taken out of service. A) Queen of Hearts (Landside Dry Dock): Electrical cables being that were damaged, worn or frayed were not removed from service exposing employees to the hazard(s) of electrical shock and electrocution. B) New Construction Vessel (Inside of hanger): Electrical cables being that were damaged, worn or frayed were not removed from service exposing employees to the hazard(s) of electrical shock and electrocution. C) New Construction Vessel (Outside of hanger): Electrical cables being that were damaged, worn or frayed were not removed from service exposing employees to the hazard(s) of electrical shock and electrocution. D) Equipment Garage (Main Building): Electrical with broken grounding prong were left connected and routed through standing water that was frozen over exposing employees to the hazard(s) of electrical shock and electrocution. Condition observed on or about 23JAN20 NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
