29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Throughout the facility: On or about November 26, 2017, and at times prior to, employees used the Toyota sit-down type LP-gas forklift, model 7FGCU25, SN #71854 with a personnel platform to lift employees to perform maintenance tasks. Employees were exposed to falling from distances up to 6-feet to the concrete below. The manufacturer did not approve the use of the platform.
