29 CFR 1910.254(b)(4)(iv): Terminals for welding leads were not protected from accidental contact by personnel or metal objects: On or about June 13, 2017, in the main plant at weld shop, employees were using numerous welders and the welding leads were not covered. The employees are welding various metals for bridge structures. Employee exposed to the hazard of an electrical shock. Welders with unprotected leads such as, but not limited too: a) Miller Welder is model MP -65E, serial HE781063 the written numbered #295. b) Miller Welder model MP-65E, the written number was #95 or (85)-477. c) Miller / Deltaweld 452, the written number on it was #258.
