29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: a) 9000 River Rd, Delair, NJ, 08110- Foundry: Employees were operating a forklift with a ram rod attachment which, was manufactured in house. The employer failed to get the forklift manufacturer's approval in writing. The attachment did not contain a nameplate or any markings in place, observed 1/24/2017. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
