29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: The following violations were observed on or about June 20 and 21, 2016: a. In the Corn Process Room, elevated area, where employees were exposed to a caught-by hazard of a projecting shaft end when operating the mixer without guarding the shaft by a non-rotating cap or safety sleeve. b. In the Flour Production Room were employees were exposed to a caught-by hazard of a projecting shaft end when operating the mixer #2 without guarding the shaft by a non-rotating cap or safety sleeve.
