Text For Citation: 01 Item/Group: 011 Hazard:
29 CFR 1910.178(p)(1): If at anytime a powered industrial truck is found to be in need of repair, defective, or in anyway unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: (a) On or about July 7, 2016, the employer failed to ensure that powered industrial trucks not in safe operating condition were taken out of service. Employees were exposed to serious struck by hazards in that powered industrial trucks showing the following defects were in operation: (1) Daewoo Model GC25E3 S/N: CV-004817: Brake pedal cover was missing (2) Clark Model C500-Y1300 S/N: Y1015-105-2791: Brake pedal cover was missing, tilt cylinders are leaking Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
