29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Chrome mill: The point of operation of the cylinders where material is hand fed to make rubber products was not guarded to prevent employee's hands from the in-going nip point. On 5/6/16 while hand feeding rubber into the chrome mill an employee's left hand was pulled into the nip point resulting in the amputation of four fingers. b) Mill 3: The point of operation of the cylinders where material is fed to make rubber products was not guarded to prevent employee's hands from the in-going nip point, on or about 6/2/16. c) Kobe mill: The point of operation of the cylinders where material is fed to make rubber products was not guarded to prevent employee's hands from the in-going nip point, on or about 6/2/16. d) Mill 1: The point of operation of the cylinders where material is fed to make rubber products was not guarded to prevent employee's hands from the in-going nip point, on or about 6/2/16. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
