Text For Citation: 01 Item/Group: 011 Hazard:
29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by pinch points, in-running nip points and rotating parts: A. Baler Area: On, or about, August 18, 2015, an employee was observed operating Baler #6, a MAPV vertical hydraulic baler, while the access door to the loading chamber was open. The interlock for the access door had been bypassed. B. Shredder: On, or about, August 18, 2015, the inclined feed conveyor that fed scrap into the SSI Shredder was not adequately safeguarded in that: 1. An emergency stop device was not provided at the finish end near the rolling stair landing. 2. The on/off control was a snap switch that was not protected against accident contact and was not labeled as to its function.
