29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: The machines point of operation was not guarded and exposed employees to struck-by and caught-between hazards in the following instances: (a) On March 16, 2015, in the EPTD Press Area of Building #1, the point of operation on the Bliss Press #112 (Model No. MI/DAC 0016/Serial 0123 was not guarded. (b) On or about March 17, 2015, in the EPTD Press Area of Building #1, the finger guard to the Minster Piece Maker Press #51 (Model P2-60-36/Serial P2-60-23830) was by-passed by setting the sensor to the side allowing the machine to operate with the finger guard gate in the raised position. (c) On May 20, 2015, in the Sanding/Deburring Area of Building #2, the point of operation on Jet Combination Belt & Disc Finishing Machine (Model No. J-4200A, Serial No. 140417739/2850 S.F.P.M) was not guarded.
