29 CFR 1910.119(j)(4)(iv): The employer did not document the date of inspection, the name of the person performing the inspection, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection, and the results of the inspection for each piece of process equipment: a) The employer has not completed and implemented the Valve List Log identified in pump, compressor, piping & valve and other preventative maintenance work orders Exercise and Lubricate Isolation Valves/ Exercise and Lubricate Control Valves sections. The employer does not have documentation that each isolation and control valve has received its specified preventative maintenance and inspection.
