29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws. On or about October 31, 2014, and at times prior thereto, flexible cords/cables were connected to devices where the strain relief was broken subjecting the pull to being directly transmitted to joints and/or terminal screws. Employees were exposed to electrical shock hazards in the following instances: a) Row E, Machine #49, strain relief was broken on the cable coming out of KONA Hot Runner Systems (Model MF5, 220V/43.5AMP) Mold Temperature Control Box. b) Row D, Machine #29, strain relief was broken on the cable coming out of the 120V NOVATEC Self-Contained Loader Control Box. c) At the Material & Dryers Area, strain relief was broken to the cable at the plug-in and also leading into the 120V NOVATEC Vacuum Loader Box. d) In the Tool Room, Strain relief was broken at the plug-in of the cable coming from the 220V motor to the South Bend Lathe Machine.
