29 CFR 1910.146(f)(13): The entry permit that documented compliance and authorized entry to a permit space did not identify equipment, such as personal protective equipment, testing equipment, communication equipment, alarm systems, and rescue equipment to be provided: Paper Machine #9- On or about December 2, 2019, employees working inside the Yankee dryer were not protected from burn, asphyxiation, and entrapment hazards where the employer did not identify on the permit the personal protective equipment and rescue equipment utilized by the entrants to the permit required confined space.
