Vortex Recycling On or about 8/29/12 and at times prior thereto, personnel operate Yale forklifts with forklift attachments. These attachments included man basket lift platform, elevated man basket lift platform (1220 PDB), and a jib boom style attachment. Company has not performed any special attachment training. B.)
