29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Jobsite - On or about April 2, 2021 an employee installing shingles on the roof did not have the attachment point of his fall protection in the center of his back and instead had a rope tied around his waist.
