29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves: On or about July 27, 2017, at the beamline inside conveyor/rack system, the rotating unprotected shaft was projecting out more than half the diameter of the shaft. Employees work and walk by the conveyor with the rotating projected shaft. Employees exposed to the hazard of being caught-in the rotating protruding shaft.
