29 CFR 1910.333(a)(2): Where exposed live parts were not de-energized, other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a/ The employer is failing to protect employees from arc flash and or electrocution hazards. This condition was last observed on and about 05/05/16, at 16051 MO-HWY 47, Marthasville, MO 63357. Inside of Building #1, three General Electric 60 AMP Disconnect Boxes are installed on a live, 480 volt, Middle Row Bus Duct. Safety-related work practices are not used to protect the employee from this hazard.
