29 CFR 1910.334(a)(3)(i): A flexible cord used with the type of ground equipment did not contain an electrical equipment grounding conductor. a. Door 1 - On or about December 20, 2022, the dock light cord used to facilitate inspection of the trailer and offloading of materials was missing the grounding conductor (pin), employees were exposed to an electrical hazard. b. Between Door 5 and 6 - On or about December 20, 2022, the dock light cord used to facilitate inspection of the trailer and offloading of materials was missing the grounding conductor (pin), employees were exposed to an electrical hazard. c. Door 16 - On or about December 20, 2022, the dock light cord used to facilitate inspection of the trailer and offloading of materials was missing the grounding conductor (pin), employees were exposed to an electrical hazard. d. Door 22 - On or about December 20, 2022, the dock light cord used to facilitate inspection of the trailer and offloading of materials was missing the grounding conductor (pin), employees were exposed to an electrical hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
