29 CFR 1910.146(e)(2): Before entry began, the entry supervisor identified on the permit did not sign the entry permit to authorize entry: Boiler House- On or about November 18, 2019, employees working inside the #2 Boiler Ductwork were not protected from asphyxiation and entrapment hazards where the employer did not have the entry supervisor sign the entry permit for the permit required confined space.
