29 CFR 1910.119(g)(1)(i): The employer did not train each employee before being involved in operating a newly assigned process in the operating procedures as specified in paragraph (f) of this section: On or about July 23, 2019, and at times prior thereto, the employer did not ensure employees were trained in procedures for normal operations, emergency operations and consequence of deviations. The employer processes over 10,000 lbs of an unscented flammable propane mixture for the production operation. Employee(s) were exposed to the hazards associated with propane and butane such as burns, explosion and asphyxiation.
