29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Glue Mixing Area: On or about May 9, 2018 and at times prior to, an employee was exposed to Formaldehyde vapor above the OSHA Permissible Exposure Limit and the OSHA Short Term Exposure Limit and was required to wear a filtering facepiece respirator and the employer did not ensure a medical evaluation was performed.
