29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi. (a) Maintenance Shop: On or about June 25, 2015, and times prior, employees use burning hoses that were not pressure tested to at least 300 psi after repairs were made. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM
