29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation: (a) On or about March 31, 2015, in the fabrication shop, the employer did not ensure that employees were protected from welding cables with exposed bare conductor. Employees were exposed to electrical hazards.
