29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly A.) Vortex Recycling - On or about 8/29/12 and at times prior thereto, forklift operators have not received training on the fork lift attachments. Personnel operated Yale and Hyster forklifts with forklift attachments. These attachments included but were not limited to man basket lift platform, elevated man basket lift platform (1220 PDB), jib boom attachment and rotator attachments.
