29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices for employees and contractor employees to provide for the control of hazards during operations such as lockout / tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: ALON USA, LP, 200 Refinery Road, Big Spring, TX. The employer does not ensure that their safe hot work practices are always provided to contractor employees. This violation was observed on or about April 15, 2009 in the following instance: Alon USA does not provide its hot work procedures to Conex International employees which were conducting hot work in the SCOT Tail Gas Unit. The Conex fire watch was not aware of Alon USA Hot Work Requirements outlined for fire watch personnel in accordance with Alon's Hot Work Procedure #201. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking in the workplace to ensure development and implementation of safe work practices for employees and contractor employees to provide for the control of hazards during operations such as lockout / tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel.
